INTERNATIONAL NATURAL HISTORY: THE NATURAL HISTORY OF THE SUNDA SHELF

I hereby request a slot in BIOL 98C at Victor Valley College.

I understand that this 3-unit course will take place in Borneo and Peninsular Malaysia from 16 January – 2 February, 2005. I understand further that the price of $2900 includes roundtrip airfare from Los Angeles, transportation, lodging (in shared double rooms), meals, tours, and entry fees. Additional items, such as course tuition, transportation to/from LAX, prescriptions, personal effects, and travel insurance, as well as general costs associated with travel (e.g., passport, clothing, luggage, snacks, souvenirs, entertainment, photography, etc.) are my responsibility.

By submitting this form I am formally registering for this trip. To validate my registration, I will submit a $500 deposit within 7 days of form submission. I am aware that his deposit becomes nonrefundable on 15 November. I will submit a photocopy of my valid passport by 15 November, and I will pay the remaining $2400 on or before 01 December.
