Online Services | DSPS

This page is set up D S P S program participants to access required forms and provide direction on how to receive certain accommodations. Students using the resources on this page must complete the following prior to using these forms. (For home bound, in district students with disabilities, who are registered in on-line courses, please contact the D S P S office at (760) 245-4271 extension 2212)

 

  1. Complete a D S P S Application. Click here to download the on-line D S P S Application form.

  2. Completed screening and documentation verification by a counselor or D S P S Specialist

  3. Completed eligibility appointment with a D S P S counselor

  4. Attended an orientation and workshop for the recommended accommodation(s).

Every D S P S program participant receives reasonable accommodation according to their specific documented disability that is directly related to their educational limitation. Some forms may not apply to certain students in the D S P S program. The page is divided by tables. The left side table describes the services and the right side table links to the appropriate form.

Alternative Text 

Students who have received alternative text as an accommodation are required to complete the alternate media request. Student please examine the student handbook for the procedure. Download the Alternative Text Services Request Form

 

Alternative Testing

Students who have received alternative testing as an accommodation are required to complete the alternative in the D S P S office. The Alternative Testing Services Request Form is not available online. Please contact the Disable Students Programs and Services Office at 760-245-4271, extension 2212 to request this accommodation.


Note taking Service

Students who have received a note services as an accommodation are required to complete the request and submit it to a specialist. The form may be downloaded and mailed or email to a D S P S specialist. Student please examine the student handbook for the procedure.  Download the Note-taking Services Request Form


Interpreter Service

Hearing Impaired students who have requested an interpreter for an accommodation are required to complete the Interpreter Service request. Student receiving this accommodation must meet with the lead interpreter, D S P S specialist, and counselor to review the procedure. Download the Interpreter Services Request Form


Mobility Service

Physically challenged students who have received the mobility service as an accommodation are required to complete the Mobility Service request and submit it to a specialist. The form may be downloaded and mailed or email to a D S P S specialist. Student please examine the student handbook for the procedure. Download the Mobility Services Request Form

 

Adaptive Technology and Devices Service

Physically and learning challenged students who have received the adaptive technology service as an accommodation are required to complete the A C T Service request and submit it to the Instructional Aid III. The form may be downloaded and mailed or email to the D S P S Instructional Aid III. Other Forms

These forms require a Victor Valley College D S P S counselor's input to ensure the forms are completed and approved. Students attempting to use these forms on-line will need pre-approval contact the D S P S offices for more information. Download the Services Request to Counselor Form
