Do you have a code showing on your “My Documents” section in WebAdvisor, but you don’t know what it means? 

The following list describes the meaning of each code:

FAC11APL     11/12 Institutional Application- This form has been deleted.  We no longer require it.

FAC11APP     2011APPEAL GRANTED

FAC11DC       DEPENDENCY CHANGE

FAC11DEN     PETITION DENIED

FAC11SUS     FAILURE TO MEET SAP STANDARDS

FAC11DIS     FINANCIAL AID DISMISSAL NOTIFICATION

FAC11ISR    11/12 ISIR RECEIVED – The Financial Aid Dept. has been notified that you have chosen VVC as your school on your FAFSA. We have received your student aid report from the Department of Education.

FAC11CIT    CITIZENSHIP – Please provide documentation of your citizenship status.

FAC11COD- COD REJECT- Please come to office to resolve

FAC11DEF    DEFAULTED LOAN-You have defaulted on a student loan. You must make satisfactory payment arrangements before further financial aid can be granted. A letter of good standing is required.

FAC11DHS  DEPT. OF HOMELAND SECURITY – Please provide documentation of your current immigration status.

FAC11DRG  DRUG CONVICTION – Please call 1-800-433-3243 for assistance in determining your eligibility for federal aid.

FAC11NSL  NSLDS MISMATCH – Your name/social security number/date of birth do not match. Please provide a copy of your social security card and driver’s license or I.D. card.

FAC11PTX  2010 PARENT’S SIGNED TAX RETURN – Before we can complete your financial aid file, we need a copy of your parent’s signed 2009 tax return.

FAC11PW2  2010 PARENT’S W-2 – Before we can complete your financial aid file, we need a copy of your parent’s W-2’s.

FAC11SEL  11/12 SEL SERVICE MISMATCH – Please verify selective service registration.

FAC11STX  2010 STUDENT’S TAX RETURN – Before we can complete your financial aid file, we need copies of your 2009 tax return.

FAC11SW2  11/12 STUDENT’S W2 – Before we can complete your financial aid file, we need copies of your 2009 W-2’s.

FAC11OVR  OVERPAYMENT – You have received an overpayment of financial aid funds from the Department of Education, and must make satisfactory repayment arrangements before you can receive further financial aid.

FAC11DVR   11/12 DEPENDENT VERIFICATION WORKSHEET – Before we can complete your financial aid file, you must complete a Dependent Verification Worksheet.

FAC 11IVR  11/12 INDEPENDENT VERIFICATION WORKSHEET – Before we can complete your financial aid file, you must complete an Independent Verification Worksheet.

FACXXBC  BIRTH CERTIFICATE – Before we can complete your financial aid file, you must submit a copy of your birth certificate.

FAC11ATB  ABILITY TO BENEFIT – Before we can complete your financial aid file, you must take the Ability to Benefit test, and submit your scores to the Financial Aid Office. Under certain circumstances, previous units completed may be used in lieu of test scores.

FAC11ATT   OVER 90 ATTEMPTED – You must petition for financial aid, as you have attempted over 90 units. Please see our SAP (Satisfactory Academic Progress) policy.

FAC11CAH   CAHSEE EXIT EXAM – Please submit a copy of your Cahsee Exit Exam results to the Financial Aid Office.

FAC11BOX   INTAKE VIA RED BOX- Paperwork submitted via the red box.

FAC11DEG  AA/AS DEGREE – You must petition for financial aid, as you have earned an AA/AS Degree. Please see our SAP (Satisfactory Academic Progress) policy.

FAC11HSD   HIGH SCHOOL DIPLOMA – Before we can complete your financial aid file, you must submit a copy of your high school diploma.

FAC11VID  I.D. OR D.L. – Please submit a copy of your valid photo I.D. or Driver’s License to the Financial Aid Office.

FAC11SSC   SOCIAL SECURITY CARD – Please submit a copy of your Social Security Card to the Financial Aid Office.

FAC11NC  NAME CHANGE – Your name does not match in Financial Aid and Admissions and Records. Please provide a copy of your social security card and ID that match to both departments.

FAC11PET 11/12 ACADEMIC/PROGRESS APPEAL - You must attend a SAP workshop to receive an appeal form.  View SAP at: http://www.vvc.edu/offices/financial-aid/sap.shtml.

  • Degree - Has degree - requesting extension
  • Dismissal - GPA below 2.0 or completion ratio below 67%
  • Max time frame - exceeds units or time frame

FAC11PIC – PARENT INCOME CERT – Before we can complete your financial aid file, you must submit a Parent Income Certification Form.

FAC11RES  RESIDENCY CONCERN –  Please clarify your residency with Admissions and Records, and notify Financial Aid of the outcome.

FAC11SIG  FAFSA REQUIRES SIGNATURE – Your SAR (student aid report) needs to be signed before we can proceed with your financial aid file. If you completed an online FAFSA, please use your pin number to sign electronically.

FAC11SIC  STUDENT INCOME CERT – Before we can complete your financial aid file, you must submit a Student Income Certification Form.

FAC11UN  UNTAXED VERIFICATION FORM – Before we can complete your financial aid file, you must submit an Untaxed Income Verification Form.

FAC11HAR HOMELESS AT RISK FORM - Submit official documentation from the Director of a Runaway or Homeless Youth Basic Center, or transitional living program substantiating you were an unaccompanied youth who was homeless, or were self-supporting and at risk of being homeless after 7/1/08.

FAC11HBS  HOMELESS BY SCHOOL CERT - Submit official documentation from your high school or school district homeless liaison substantiating your  homeless determination after 7/1/09.

FAC11BA   BA/BS DEGREE - you must petition for financial aid, as you have earned an AA/AS Degree. Please see our SAP (Satisfactory Academic Progress) policy at: http://www.vvc.edu/offices/financial-aid/sap.shtml

FAC11CPL  11/12 VA CHANGE OF PLACE- submit a VA form 22 1995 (chapter 30&33) or VA form 22 5495 for chapter 35

FAC11EDP   EDUCATION PLAN- please submit a current educational plan.

FAC11EMM EMANCIPATED MINOR- provide court documents of emancipation.

FAC11ENT  11/12 ENTRANCE LOAN COUNSELING- complete the Entrance exam at this website:  https://studentloans.gov/myDirectLoan/index.action

FAC11EXT  LOAN EXIT COUNSELING- complete the Exit counseling at this website  http://www.nslds.ed.gov/nslds_SA/

FAC11FAL   FALL PROBATION STATUS- your academic/progress status is being monitored

FAC11FLT    11/12  VA F LETTER- you must respond to your VA “F” letter.

FAC11GEP   11/12 GI BILL EDUCATION PLAN- have a counselor complete your VA educational plan and submit to FA.

FAC11LEG     LEGAL GUARDIANSHIP-provide court papers on your legal guardianship.

FAC11LWR     11/12 LOAN WORKSHOP-complete the loan workshop, check FA website for dates and times.

FAC11MA        MA/MS DEGREE- ineligible for Pell grants SEOG, master’s degree reported.

FAC11MIS       MISC

FAC11MPN     11/12 LOAN MASTER PROM NOTE- complete the loan master promissory note @: https://studentloans.gov/myDirectLoan/index.action

FAC11OAL       11/12 OVERAWARD LETTER-over award reported.

FAC11OWC     ORPHAN OR WARD OF THE COURT-provide court papers to FA office.

FAC11R2T       11/12 RETURN TO TITLE IV- you may owe fund to Dept. of Ed.

FAC11SCH       2010 1040 TAX SCHEDULES-provide all 2010 tax schedules.

FAC11SPC       SPECIAL CONDITION- provide FA office with Special Conditions forms and supporting documentation.

FAC11SR2       11/12  R2T4 LETTER- please read carefully

FAC11TRN       TRANSCRIPTS FROM ALL SCHOOLS-provide transcripts from all schools attended.

FAC11WIN    INTAKE VIA THE FRONT WINDOW- Paperwork turned in via the front Financial Aid window.

FAC11UDM      2011-2012 UNDECLARED MAJOR- a major must be declared, correct with Admissions and notify FA.
