Applications are available online and by email from the E.O.P.S. office.
In order to become a C.A.R.E. student, one must be an E.O.P.S. student first.
You must submit an untaxed income form and a perjury form. If you are approved, you will need to submit these forms each Fall semester and Spring semester.
