This form is designed for lesbian, gay, bisexual, asexual, pansexual, transgender, gender-nonconforming, intersex, and two-spirit (LGBTQIA2S+) faculty, staff, and students to connect with the designated campus point of contact. This is a space to share incidents of discrimination, harassment, intimidation, or bullying you've experienced or witnessed, and to explore support, resources, and your options.
Please Note: This form is for connecting with your campus point of contact for support and information. Submitting this form to the designated point of contact DOES NOT initiate a formal investigation by the institution or under Title IX. The point of contact is not a "responsible employee" for formal reporting purposes. The designated point of contact cannot disclose confidential information shared with them without your prior written consent. If you wish to initiate a formal investigation with the institution (e.g., regarding discrimination, harassment, or sexual violence), you must directly contact the Vice President of Human Resources, Monica Martinez.
